Please note that this website policy is not your HIPAA Notice of Privacy Practices. 1. Services described on this website
YuWellnes provides telepsychiatry, including psychiatric evaluations and medication management, with a focus on adult ADHD, anxiety, depression, and insomnia. Care is provided virtually across Arizona, Colorado, Delaware, Iowa, Kansas, Montana, New Mexico, New York, Texas, Vermont, and Washington, and in person at our Frisco, Texas office every Saturday.
This website itself does not provide medical care. It provides information about our practice and a link to book a free 15-minute consultation.
2. Information we collect
Information you provide directly includes your name, phone number, and any message you send when you call, text, or contact us with questions. It also includes information you enter when you book through our booking link, which is operated by a third-party scheduling provider, currently OptiMantra. That provider has its own privacy policy, and we encourage you to review it before booking.
Please do not send detailed health information, insurance ID numbers, or other sensitive information by ordinary text message or email unless we have specifically asked you to use a secure method to do so.
When you visit the website, we and our website host may automatically collect limited technical information, such as your IP address, browser type, device type, pages visited, and the time and date of your visit. We may use cookies or similar technologies for basic website function and to understand how visitors use the site. You can control cookies through your browser settings, but some website features may not work if cookies are disabled.
3. How we use information
We use information collected through the website to respond to your questions and appointment requests, to schedule and coordinate care including through our booking provider, to operate, maintain, and improve the website, to communicate with you about appointments and practice information as permitted by law, and to comply with legal obligations and protect the safety and security of our practice and website visitors.
We do not sell your personal information.
4. How we share information
We may share information collected through the website only as described in this policy. We may share it with vendors who help operate the website, scheduling, communications, or practice operations, and who are required to protect that information. We may share it when required by law, by court order, or to protect safety, or as otherwise permitted under HIPAA and applicable law for patient information. In other cases, we share it only with your consent or at your direction.
Our booking, insurance, and social media links, including Instagram, TikTok, and Facebook, are operated by third parties. Their privacy practices are governed by their own policies, not this one.
5. Third-party booking
Booking a free 15-minute consultation takes you to a third-party scheduling site, OptiMantra. Information you enter there is collected and handled by that provider under its own terms and privacy policy, as well as by us once it is shared with our practice for scheduling. Please review that provider's policy before entering health, insurance, or payment information.
6. Children's privacy
This website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the website. For any minor patient, a parent or legal guardian must consent to care and may be required to participate as required by law and clinical policy.
7. Your privacy rights
Depending on the state where you live, you may have rights under state privacy laws, which may include the right to know what personal information we collect, to request access to or a copy of it, to request correction or deletion, and to opt out of certain uses. Texas residents have rights under the Texas Data Privacy and Security Act, and residents of other states we serve may have similar rights under their own state laws.
To exercise a right, or to ask a question about this policy, contact us at (602) 633-4667 or (602) 633-4655, or by mail at the address above. We will verify your identity before responding to a request about your information, and we will respond within the time required by applicable law. You will not be discriminated against for exercising a privacy right.
8. Security and retention
We use reasonable administrative, technical, and physical safeguards to protect information collected through the website. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security.
We keep website contact and scheduling information only as long as needed for the purposes described in this policy, for patient care and practice records where applicable, or as required by law.
9. Emergency information
This website is not for emergencies. If you are experiencing a medical or mental health emergency, call 911 or go to your nearest emergency department. In the United States, you can also call or text 988 for the Suicide and Crisis Lifeline.
